ADVANCE CARE PLANNING

Advance Care Planning (ACP) involves making decisions about your healthcare in advance, ensuring your preferences are known if you cannot speak for yourself.


This process includes discussing your medical treatment and life-sustaining choices with family, loved ones, and healthcare professionals.

ACP aims to guarantee that your wishes are understood and honoured, particularly in critical or end-of-life circumstances. My role is to support and assist. I do not provide legal, financial, medical, or estate planning advice, nor do I make decisions for clients. I want to ensure that my client’s wishes are respected before, during and after their death.

Key elements of advance care planning that I will help facilitate:

Providing Emotional Support.

Beyond practical planning, I provide emotional support. I assist my clients in exploring their values, concerns, and wishes for their final days, ensuring these are incorporated into the care plan.

Guiding the Process.

I assist my clients by explaining important documents, such as living wills, roles of substitute decision makers, durable powers of attorney for healthcare, and do-not-resuscitate (DNR) orders. I offer practical guidance so that my clients fully understand their options and the implications of each document in their province.

Providing Resources.

I provide valuable resources such as information on hospice care, home hospice care, palliative care, and other end-of-life-services.

End-of-life planning empowers clients to gain control over their future and have peace of mind knowing their affairs are in order.

As my client's advocate, my goal is to empower them and ensure their wishes are respected at every step.

Key elements of end-of-life care planning that I will help facilitate:

Creating personalized end-of-life plans.

I inform my clients of their rights and options.

Clarifying the client's preferences.

I work alongside my clients to facilitate their wishes in areas such as funeral service arrangements, celebration of life, memorial services, burial, or cremation, and ensuring these are communicated to their loved ones.

Organizing personal affairs.

I support my clients in sorting out their matter to alleviate difficult situations for their loved ones after death. This could include writing final letters, distributing personal items, advance directives, and documentation, or anything the client wishes to simplify & resolve.

Services & Associated Fees

First Steps:

The first step is to get to know one another. I offer new clients a complimentary 30-minute phone consultation. This is an opportunity for you to ask questions, express concerns and wishes, and for me to understand your unique situation and needs. We can discuss and determine how I can best support you. I will follow up after our initial conversation within a few days.

Fees:

Fees are determined based on the customized approach tailored to each client's unique needs. End-of-life support is highly personal. It is your journey with me at your side. I will collaborate with you to determine the most suitable plan, whether it's on an hourly basis or as a part of a session package. Additionally, I offer a sliding scale for clients facing financial limitations.

Payments & Cancellations:

Fees are due at the start of our services, and can be paid via e-transfer or cash before we begin working together. Clients will receive receipts for all services provided.

I kindly request 48 hours notice for cancellations. If less than 48 hours notice is given, clients will be charged the full fee, except in emergencies.

I welcome individuals and families of all ages and backgrounds who are seeking compassionate, personalized support in navigating end-of-life decisions, care planning, legacy work, and emotional transitions. My services are available to all, regardless of age, background, beliefs, or circumstances.

Where I Work:

As an end-of-life consultant, I bring compassionate care to a variety of settings, including homes, hospices, hospitals, long-term care facilities, and retirement homes. I meet each client where they are, providing personalized support that respects their unique needs and circumstances. This includes offering services online if the client prefers.
